Low testosterone questionnaire for men

The Aging Males' Symptoms (AMS) scale is a tool to help men assess the symptoms of aging.1 
To complete the questionnaire, men should rate how they experience key symptoms, such as low sex drive and difficulty sleeping, on a scale of 1 (none) to 5 (extremely severe).

Men can then download and print off their AMS results and take them to an appointment with their healthcare provider to inform conversations about their symptoms.
